As an authorized Club Representative I agree to be responsible for ensuring that parents and supporters of this club attending Field Hockey Ontario (FHO) events act in accordance with FHO policies and Codes of Conduct as published, and revised from time to time, on FHO website; and that conduct that contravenes the Codes may result in sanctions to the Club under the FHO Complaints /Discipline Policy up to or including having Club recognition rescinded

Photo Consent- I authorize Field Hockey Ontario, Field Hockey Canada and/or their staff, associates, assistants, or subcontractors to use photograph(s), video and/or other media images of myself or the named minor captured at a Field Hockey Ontario or Field Hockey Canada event for any promotional, educational or other pertinent uses. These images may include, but are not exclusive to, club, coach, athlete and/or parent, and/or volunteer submissions for contests, Field Hockey Ontario or Field Hockey Canada websites, and any of their various reports. I authorize Field Hockey Ontario and Field Hockey Canada to permit the use and display of photographs and/or recordings of the named minors in any Field Hockey Ontario or Field Hockey Canada publication, multimedia production (including video and web usage), display, or advertisement. I agree that Field Hockey Ontario and Field Hockey Canada may use name, likeness, or information supplied by me. I release and forever discharge the Field Hockey Ontario, Field Hockey Canada, their agents, officers and employees from any and all claims and demands arising out of or in connection with the use of said photographs / recordings, including but not limited to, any claims for invasion of privacy or defamation.
